At Tellegacy, we’ve long understood that connection is care. Since our founding, we’ve been committed to reducing social isolation among older adults through intergenerational programs that bring students and seniors together in meaningful dialogue. Now, we’re taking the next step.

We’re excited to announce that Tellegacy is soon to launch a new service line: community-based case management, specifically designed to support older adults enrolled in Medicare.

As we expand our impact, we’re inviting mission-aligned organizations, healthcare partners, and community leaders to collaborate with us. If you or your organization serve aging adults—especially those who could benefit from consistent, values-driven case management—we’d love to hear from you.

Why Case Management?

For many older adults, the healthcare system can feel overwhelming, fragmented, and impersonal. Case managers act as advocates, guides, and connectors—ensuring that older adults:

  • Follow their care plans
  • Access the services they need
  • Avoid unnecessary emergency visits or hospital readmissions
  • Feel emotionally supported in the process

Tellegacy case managers go a step further by integrating our signature intergenerational approach with emotional intelligence, SDOH, Age-Friendly Care, cultural humility, and deep listening.

The Problem We’re Solving

Older adults—particularly those with chronic illnesses, cognitive challenges, or limited social support—are at significant risk of falling through the cracks. Many health plans and providers are seeking innovative, value-based strategies that reduce costs while improving quality of life.

Tellegacy case managers are trained to address the social determinants of health (SDOH) that so often drive poor outcomes. Our model is designed to align with the goals of:
