At Tellegacy, we believe the best care starts with genuine connection. That’s why we’re launching a new initiative—and we’re looking to partner with a compassionate Nurse Practitioner who is not only seeking another income stream but also desires to be part of something deeply meaningful.

In this chapter of our work, Tellegacy is expanding its mission by collaborating with universities across the country to provide credentialed students and alumni—especially Nurses, Social Workers, and other healthcare professionals—with practical opportunities to serve in one of the most critical and overlooked areas of healthcare: transitions of care.

Research shows that transitions—from hospital to home, assisted living to nursing home, or home to hospital—are moments where older adults are particularly vulnerable. These shifts can determine whether someone thrives or struggles. Unfortunately, too many older adults experience these transitions without proper support. We aim to change that.

What Tellegacy Offers:
Our program goes beyond routine care. We train students and professionals to provide Medicare-compliant services while practicing deeper engagement. Each participant will help older adults share their stories, reflect on their lives, and document “What Matters” to them in a professionally crafted legacy book. This is more than a keepsake—it’s a tool to elevate care by aligning health support with the individual’s values and lived experiences.

Each book will include a personalized reflection of how the individual’s values relate to the Social Determinants of Health (SDOH)—a powerful way to help healthcare teams tailor their care and understand the whole person. Because when connection improves, care improves.
